The Community Solutions' Family Wrap program is a menu of evidence-based family support services designed to prevent children from entering the foster care system and keeping families together. Each family situation is unique and requires a holistic approach based upon a real-time assessment. Community Solutions (CS) begins services where the Child Protective Services (CPS) assessment ends. If CPS has determined that the child can safely remain in the home, CS will partner with the Department of Social Services to quickly enter the home and stabilize the family system. The following factors are evaluated by the CS Assessment Team:
Based on these 8 factors CS develops a Family Support Plan designed to create a safe, stable and healthy environment in the home. CS uses a variety of approaches and tools to address challenges and obstacles identified during the assessment process.

- Family history of trauma, abuse, neglect.
- Parental capabilities and skills.
- Degree of alcohol and other substance use/abuse in the home by parents, other relatives living in
the home or adolescent children. - Involvement with the juvenile or adult criminal justice system.
- The physical health and mental health status of both adults and children living in the home and the
present participation in treatment to address these challenges. - The economic challenges facing the family and the stress level related to these challenges.
- Housing status. Does the family reside in safe and stable housing?
- Strength of a family support system.
Based on these 8 factors CS develops a Family Support Plan designed to create a safe, stable and healthy environment in the home. CS uses a variety of approaches and tools to address challenges and obstacles identified during the assessment process.

Four Family Wrap Stages:
Stage 1: Engagement and Assessment – The first phase is designed to form an alliance with the family and develop initial trust to move forward and identify Individual and family goals
Stage 2: Motivation, Education and Resource Linkage – Family Mentors work closely with family members to identify strengths, challenges and opportunities for positive change. The primary goal of this phase is to connect family members to people, places and things that will support them in reaching their individual and family goals.
Stage 3: Generalization: Life After CS – As the family develops the knowledge and skills to achieve success and the connections to positive role models and resources, CS Mentors function more as a consultant giving feedback to the family members to ensure they continue to progress in implementing the plan on their own.
Stage 4: Aftercare and Outcomes – When the CS Wrap intervention is concluded, CS checks in with the family on a monthly basis for a one-year period to determine if the family is following through on their Family Plan and gather outcome data.

Community Solutions has developed an extensive network of community partners to help assist families. Our partners address the educational, health-related and financial challenges facing our referred families. The Resource Map is a database that assists clients in identifying the best option for assistance and support in their home community in every area of the family's need.
